Thank you for inquiring about a PPP loan with The Hicksville Bank!  Our commitment to you is to provide a timely and accurate review of your information, as we understand how important these funds are to your business operations. 

By providing some preliminary information on the below fields, we may begin the process of determining eligibility.


Please note that the SBA deadline for loan approval is presently March 31, 2021.  We will do our best to process your application as quickly as possible.  However, we cannot guarantee your loan application will be processed by SBA prior to the March 31, 2021 deadline.

Do/did you take a First Draw PPP Loan?

Upon receipt of this information, you will be receiving an email from our servicing agent, BOD Financial, including a secure link to apply for the PPP loan. You may need the following documentation in order to complete the online application:

  • 2019 and/or 2020 Federal Tax Returns, including all schedules.
  • 2020 signed financial statements.
  • IRS Form 1099 Misc. detailing Nonemployee compensation received.
  • Bank Statement from December 2020 or earlier or Book of Record establishing self-employment.
  • State Quarterly Wage Unemployment Insurance Tax Reporting for 2019 and/or 2020.
  • 2019 and/or 2020 Payroll Records
  • Evidence of Health Insurance Contributions or any retirement contributions.
